At Life Solutions OT, we provide occupational therapy assessments to support older Australians to live safely and independently at home under the new Support at Home program.


We partner with registered Support at Home providers to deliver professional assessments and recommendations focused on home safety, falls prevention, assistive technology, and minor home modifications. Our team also completes modification diagrams, liaises with builders, and obtains quotations to support timely and compliant installations.


Through these collaborative services, we help clients and providers identify practical solutions that enhance safety, mobility, and confidence within the home environment.

Assistive Technology Solutions

We prescribe and recommend assistive technology to further support independence in daily activities. These solutions are tailored to meet individual needs and assist with mobility, transfers, self-care, and home management. Assistive technology includes:


Mobility aids
Electric recliners & beds
Transfer aids
Self-care aids, including dressing aids (sock aids, shoe horns, button aids) and washing aids (long-handled sponges).
Kitchen aids, such as adaptive cutlery, electric can/jar openers, tap turners, kettle tippers, kitchen stools, and meal tray walkers.
Bathroom & toilet aids, including shower chairs, shower stools, toilet surround frames, toilet seat raisers, non-slip mats, and bath boards.


These solutions enable older adults to complete their daily tasks with greater ease, safety, and independence, reducing reliance on carers and improving overall quality of life.

Home Modifications For Older Australians

At Life Solutions OT, we recommend minor home modifications to enhance safety, accessibility, and independence for older adults. Our modifications are designed to reduce fall risks, improve mobility, and support safe daily living. These include:

Home Safety Assessments For Falls Prevention

Falls are one of the biggest risks to older adults, often leading to serious injuries and hospitalisations. Our home safety assessments focus on identifying hazards and assessing how clients move around their home, particularly in high-risk areas such as the bathroom, bedroom and lounge/dining room.


We provide recommendations for non-slip mats, grab rails, optimal lighting and clutter reduction to minimise trip hazards. Additionally, we assess how individuals engage in activities such as getting in and out of bed, showering, dressing, and moving through their home, offering strategies and assistive devices to improve safety and confidence in daily living.

Referral Process & Service Delivery

Life Solutions OT provides occupational therapy services for older Australians receiving funding through the new Support at Home program.


As we operate as an associate provider, we subcontract under registered Support at Home providers and are not directly registered ourselves. This means we can no longer accept referrals directly from clients. Instead, your registered Support at Home provider or case manager will need to make the referral on your behalf.


We work in partnership with multiple aged care providers to ensure older Australians can access high-quality occupational therapy services tailored to their needs. If you are unsure about the referral process, please speak with your Support at Home provider or case manager, who can guide you in arranging an assessment with us.
